You choose where your prescription is filled
If a BioForm Rx provider prescribes medication for you, you have the right to have that prescription filled at any licensed pharmacy you choose. You are never required to use a pharmacy we recommend, and your care does not depend on which pharmacy you select.
The pharmacies we work with
BioForm Rx works with a small number of licensed compounding pharmacies to fill the prescriptions our providers write. Depending on the medication, your state and pharmacy availability, your prescription may be sent to BioForm Compounding LLC or to one of our partner pharmacies, each of which is licensed to dispense in your state. We tell you which pharmacy is filling your prescription before it ships, and the pharmacy’s name and contact information appear on your medication label.
Our relationship with BioForm Compounding
BioForm Rx is a DBA of The Tide Clinic PLLC. BioForm Compounding LLC, a Texas compounding pharmacy, is under common ownership with the management company that provides business services to The Tide Clinic PLLC. In plain terms: the same owner has a financial interest in both the clinic that treats you and one of the pharmacies we recommend.
We disclose this so you can make an informed choice. We work with BioForm Compounding and our partner pharmacies because we know how their medications are prepared, tested and shipped, and because we have negotiated pricing with them that we can confirm for you in advance.
What our providers are paid for
Our providers are compensated for the medical care they deliver. They receive no payment, bonus or other benefit based on which pharmacy fills your prescription or on how many prescriptions are sent to any pharmacy. Every prescribing decision is made independently by a licensed provider based on your evaluation, your medical history and your lab results.
How to use a different pharmacy
You can select your preferred pharmacy during intake, or change it at any time by messaging your care team through the patient portal or emailing support@bioformrx.com. We will send your prescription electronically to the pharmacy you name, provided it is licensed to dispense the prescribed medication in your state. If your chosen pharmacy cannot fill a compounded medication, we will tell you and help you find one that can.
What you pay
Your BioForm Rx plan fee covers your provider evaluation, follow-up care and monitoring. Medication is priced separately.
If you use BioForm Compounding or one of our partner pharmacies, your medication is billed at the pricing we have negotiated with that pharmacy, which we confirm before your prescription is sent. If you choose a different pharmacy, you pay that pharmacy directly at its own price, which may be higher, and any difference is your out-of-pocket cost. Your BioForm Rx plan fee does not change based on the pharmacy you choose.
