Subscription and Refund Policy

This policy explains how BioForm Rx plans are billed, how to cancel, and when refunds are available. By starting a plan you agree to this policy and to our Terms of Use.
What your plan includes
A BioForm Rx plan is a subscription to medical care, not a purchase of medication. Your plan fee covers your provider evaluation, ongoing follow-up, dose adjustments, care-team messaging and monitoring at the cadence described on your treatment page. Medication is prescribed only if a licensed provider determines it is appropriate for you, and it is billed separately by the dispensing pharmacy. Billing
- Plan fee. Your plan fee is charged when you complete intake and then automatically at the start of each billing cycle you selected: monthly, or every three or six months at the discounted rate shown at signup. The amount, cycle and renewal date are shown before you pay and in your confirmation email.
- Medication. Medication charges are separate from your plan fee and are billed when a prescription is sent to the pharmacy. Medication prices are confirmed with you before the prescription is sent.
- Labs. Lab work is billed separately at the price shown at signup and is charged when the lab order is placed.
- Auto-renewal. Your plan renews automatically until you cancel. We email you at least seven days before any renewal of a multi-month plan and before any change to your plan price. Price changes take effect at your next renewal, never mid-cycle.
Cancellation
You can cancel at any time, and cancellation is as easy as signing up: cancel from your account settings in the patient portal, or email support@bioformrx.com. We confirm cancellations within two business days. No phone call is required.
Cancellation stops future charges. It takes effect at the end of your current billing cycle, and you keep access to your care team until then. Multi-month plans cancel at the end of the prepaid period.
If you are taking a medication that should not be stopped abruptly, please message your provider before cancelling so they can advise you on a safe taper. Cancelling your plan does not cancel a prescription that has already been sent to a pharmacy.
Refunds
- If you are not approved for treatment. If a provider reviews your intake and labs and determines that treatment is not appropriate for you, we refund your plan fee in full. Lab fees are not refunded once the lab order has been placed, because the lab has already performed the work.
- If you cancel before your evaluation. If you cancel before a provider has begun reviewing your intake, you receive a full refund of your plan fee.
- After your evaluation has started. Plan fees are not refundable once a provider has begun your evaluation or a prescription has been issued, because the medical service has been delivered. If you cancel mid-cycle, you keep access through the end of the cycle and are not charged again.
- Medication. Prescription medications cannot be returned or refunded once dispensed. This is required by pharmacy law and applies to all pharmacies, including those we recommend. If a medication arrives damaged, incorrect or compromised by shipping, contact us within 48 hours of delivery with a photo and we will work with the pharmacy to replace it at no cost to you.
- Missed video visits. If your treatment requires a video visit and you miss it without rescheduling at least 24 hours in advance, one rebooking is included at no charge. Additional missed visits may be charged a $25 rescheduling fee.
- Multi-month plans. Prepaid multi-month plans are refundable on a prorated basis if a provider discontinues your treatment for medical reasons. They are not refundable if you choose to cancel early.
How refunds are paid
Approved refunds are returned to your original payment method within five to ten business days of approval. If your payment method is no longer valid, contact us and we will arrange an alternative.
Billing disputes
If you believe you have been charged in error, contact us at support@bioformrx.com before disputing the charge with your card issuer. Most billing issues can be resolved within two business days.
Changes to this policy
We may update this policy by posting a revised version with a new effective date. Changes to pricing or billing terms apply at your next renewal, and we will notify you in advance.
